REGISTRATION FORM COMPLETION INSTRUCTIONS

NASC REGISTRATION FORM
1. Before completing the form, determine your child's age group. If you do not know your child's age                               group, refer to the age group table  AGE GROUP TABLE
2. Click on the link above to launch the registration form.
3. Complete the form in its entirety.
4. Print the completed form by clicking on the 'Print' icon on the bottom of the form.
5. Sign and date the completed form.
6. If mailing the form, Address an envelope to: REGISTRAR at NASC P.O. Box 5144 New Castle, PA 16105.
7. Place the completed form and a check for the appropriate fee and place in the U.S. Mail.
8. All mail in registrations must be postmarked by August 1, 2006 Travel players and September 8, 2006 In-House players. All registrations postmarked after the listed deadline must pay late fee's.  Please check with Registrar.

 
ADDITIONAL NOTES:
  • If your child is a new player you must furnish a copy of their Birth Certificate.
  • If your child is a member of a travel team, you must furnish a 1"x 1" photograph and                                                    the player must sign a player card.
  • Completed registration forms can be dropped off in person at the Y SPORT ZONE                                                   :
  • NASC Registrations will be held at the "Y SPORTS ZONE" during practice and or games, inquire with your coach
